You agree that in exchange for your payment, you receive a DNA test kit, access to a cloud-based patient portal, and a 30-minute custom-analyzed genetic interpretation video.
If you wish to dive deeper into your interpretation or genetics, you may schedule a 1:1 consult.
All information you provide to me in your intake form is confidential. None of the information you provide on the intake form, DNA kit registration, or Patient Portal will be bought, sold, or shared to any third party.
The information that is gathered from the intake form is done so for the sole purpose of being better able to custom tailor your interpretation and recommendations. The more information fields that you complete, the more personalized your interpretation can be.
You are free to share the information I provide to you with your practitioner. You may share your interpretation video and segments of it, provided you do not modify or delete any part of it, or use any part of it to commercialize off of.
I am not a licensed medical practitioner, therefore my services do not provide any diagnosis, treatment, or prescribing of any sort. The points discussed in your genomic analysis interpretation video are for informational purposes only.
Any outside, third-party resources or links provided in the interpretation video or in correspondence are for informational purposes only and are not warranted for content or accuracy.
You should not use the information provided in your interpretation video for self-diagnosing or treating a health problem or disease. I encourage you to discuss your results with your health-care professional, as relying on the interpretation video without seeking medical advice could result in serious injury or death. Do not disregard, avoid, or delay obtaining medical or health related advice from your health-care professional because of anything stated in interpretation video.
Additionally, if you were referred to my services by a practitioner, your practitioner holds no liability for any of the information that is presented to you in the genomic analysis interpretation video.
The software that is used for your intake form is not HIPAA compliant, so if there is any information in your intake/health history form that you do not feel comfortable sharing over this platform, please email it to me at [email protected]
Your account: The purchase of your kit will provide you with a link to set up your patient portal and register your kit. If at any time you wish to terminate your account, please request this via email [email protected]
*Please note that the Functional Genomic Analysis kit is non-refundable.* It is not covered by insurance.
Refund Policy: The cost of the genetic test kit ($230) and service & processing fees ($70) are non-refundable, but should you decline interpretation services, the remaining balance will be refunded to you as long as the email that your raw data is ready has not yet been sent to you.
1:1 consultations require a full 24-hour notice of cancellation or reschedule, otherwise the full consultation fee will be charged.
Currency: All sales are billed in USD.
You agree that you are of legal age (not a minor) in your place of residence to purchase a genetic test kit and Jaclyn Downs, Functional Nutrigenomics, LLC services.
Errors in salivary DNA sample collection: If the salivary collection tube is not filled to specified level, this may result in inability to process the sample. In this event, another kit must be purchased by the consumer.
